Easy, just open Photoshop (if you do not have to download the open-source alternative called GIMP in gimp.org), and use layers to edit in another person looking midsection all torn, then mix the upper half of your fat friend on her. You can search Google Images, Photobucket, Flickr, etc. to zillions of different freaks muscle-heads with lots of different sizes, lighting / contrast and image. If you can not figure out how to work with Photoshop layers (it's a bit confusing early) to search Google for a tutorial on how to perform specific tasks.
